I have read and understood the Participant Waiver, Release of Liability, and Assumption of Risk in full. I agree to release Street Wise Canine / Rewild the Dog and its trainers from claims, including claims based on their negligence, to the extent permitted by Ontario law.

I confirm I am the legal owner of the dog and I accept full strict liability for my dog under Ontario's Dog Owners' Liability Act (R.S.O. 1990, c. D.16).

I am 18 years of age or older and am signing this document voluntarily.
